Fractura espontánea de fémur en una paciente que recibía tratamiento a largo plazo con bi-fosfonatos

Mujer de 67 años quien sufre fractura diafisiaria del fémur, con el antecedente de ingesta de bifosfonatos durante varios años para tratamiento de su osteoporosis. En la literatura desde hace relativamente poco tiempo se encuentran artículos que describen fracturas espontáneas asociadas a pacientes tratadas con bibosfonatos durante varios años consecutivos (1, 2), por un mecanismo que no es claro. Estos medicamentos bajan la actividad osteoclástica en el metabolismo óseo, disminuyendo el recambio óseo e incrementando la densidad mineral, pero se ha planteado la teoría de que la supresión del recambio óseo puede ser exagerada y predisponer a fracturas. A pesar de las asociaciones establecidas en muchos estudios, estas siguen siendo circunstanciales, se requiere continuar con las investigaciones y reportes de casos para llegar a una conclusión definitiva.Referencias bibliográficas
